Home Visiting Hybrid for High-Risk Moms
A two-generational strategy for expectant and new moms that are disconnected youth, homeless youth, impacted by incarceration, impacted by poverty and/or hunger with a child 0-5 years of age. The program uses the Parents as Teachers (PAT) program is an evidence-based home visiting curriculum that provides in-home instruction from birth through kindergarten to enhance child development, parenting behaviors and family well-being through development-centered parenting and parent-child interactions. This program provides targeted outreach and community based enrollment into the Youth and Young Adult Program.
